Agenda & Course objectives:

The most consistent element that exists in a negative patient outcome, is that the patient did not have the access to something that was needed or what they needed was not available. In other words, when a negative patient outcome takes place there is typically a patient care gap in place. Healthcare settings that want to reduce these negative outcomes, must understand how to eliminate the presence or risk of patient care gaps from their current processes. In this presentation, Lesa Schlatman will help healthcare settings understand the negative impact that patient care gaps can have on quality measures and patient outcomes. Lesa will help settings recognize the different types of patient care gaps, when or where they typically can occur, and whether or not they are present in their current care processes. She will also help facilities identify solutions to current care gap issues, and what steps to take to prevent or reduce the occurrence of future care gaps.

At the conclusion of this presentation, attendees should be able to:

  • Recognize various types of patient care gaps & when they can potentially occur
  • Understand the negative effects care gaps have on patient outcomes & quality measures
  • Determine if patient care gaps exist in your current healthcare process
  • Develop processes that engage patients in identifying care gaps & finding solutions

About the Presenter:

Lesa’s passion for nursing began as a small child helping to care for her beloved grandfather. Seeing the difference quality care could make, her internal wish to make a difference flourished. 

Her 24 years as an RN has included such items as bedside nursing; care planning & the discharge process; long term & skilled care settings; ADON & DON positions; hospital management; Director of an RHC; policy/procedure development; and regulatory & compliance oversight. 

Lesa’s vast work experience has taught her the true value of care coordination, patient centered care, and communication/collaboration across healthcare systems. Utilizing these core principles, she strives to assist healthcare settings in achieving effective population health programs and strategies
